ABOUT WYCO CARE CONNECTION
WYCO Care Connection is a community paramedicine program made possible through a partnership between Vibrant Health and the Kansas City, Kansas Fire Department. This program seeks to reduce the number of individuals using emergency medical services for non-emergent care. This program aims to reduce strain on the local health system and improve the quality of care for patients.
HOW IT WORKS
Our community paramedicine team, consisting of one nurse practitioner and one paramedic, will provide a range of basic primary and preventative healthcare services to individuals identified as frequent emergency department users. When appropriate, this team will connect these individuals with community-based primary care and social support services.
